Pico Laser for Freckles

Fragmenting surface pigment, spot by spot

True freckles sit near the surface and often respond to short pulses. The laser does not change your tendency to form new ones. Results vary.

In Brief

Pico laser for freckles uses very short pulses to break up surface pigment, usually one spot at a time. It suits true freckles that darken in sun and fade without it. It does not stop new freckles forming, and sun protection continues. Results vary.

What the Research Shows: Pico Laser for Freckles

  • A 2024 prospective study of 12 Chinese patients given one treatment reported mean freckle clearance of 74.46%; at eight weeks, one had hyperpigmentation and one had hypopigmentation. [1]
  • A 2023 randomised trial of 60 women reported week-24 MASI improvement of 35.9% with picosecond Nd:YAG, 25.5% with alexandrite and 24.0% with hydroquinone; four patients (6.8%) had recurrence. [4]
  • A 2022 randomised split-face trial, funded by Beiersdorf, of 59 patients with phototypes III to IV found no significant difference in pigmentation score between sunscreens after picosecond laser. [5]
  • A 2023 narrative review, with manufacturer-employed authors and no pooled sample, said skin of colour is predisposed to pigmentary disorders and suggested broad-spectrum high-SPF sunscreen also covering UVA1 and visible light. [6]

About This Treatment

The short answer

A pico laser fragments the melanin in a freckle with a very short pulse. The effect comes mainly from pressure rather than heat. Freckles are shallow, so a short wavelength is usually chosen.

The first step is to confirm that the marks are freckles. Sun spots, melasma, flat moles and, uncommonly, lentigo maligna can look similar. Dr Sin Yong checks them first, with a dermatoscope where needed.

How the wavelength is chosen

WavelengthWhere it may be used
532 nmThe usual choice, because freckles are superficial targets.
785 nm picosecondWhere freckles are denser or the skin is darker.
1064 nmKept for deeper pigment, or for darker skin where surrounding melanin needs sparing.

Who pico laser for freckles suits

True freckles

Spots that darken in the sun and fade without it.

Dense freckles in darker skin

Fitzpatrick IV to V skin may be treated, often with adjusted settings and a test area first.

Who should wait, or look elsewhere

Recent tan

Wait until it fades. Tanned skin absorbs more energy and darkens more readily.

Sun spots

Larger, fixed dots are solar lentigines. They are a different condition and are not treated as freckles.

Melasma

Symmetrical patches that heat provokes are melasma, which is planned differently.

Changing marks

A dot that is changing, irregular or darker than its neighbours is examined first and may be referred to a dermatologist.

What it cannot do

Pico laser does not stop new freckles forming. Sun exposure prompts them, and the laser does not change that tendency. It also cannot promise a set number of visits, because treatment may be staged. The aim is to treat the pigment present today. Daily sun protection continues, and new freckles may still appear.

What a visit involves

01
Treatment
Each freckle is usually treated as a spot. Greying or whitening appears and fades over minutes.
02
Other options
Faint or numerous freckles may be given lower-energy passes across the area, or a combination of approaches.
03
Healing
A fine crust forms and flakes off by itself. Do not pick it.
04
Review
Any further treatment waits until the crust has cleared and redness has settled.

Risks and side effects

Brown halo

Post-inflammatory hyperpigmentation can form around a treated spot. Picking, sun during healing or excess energy make it more likely.

Pale dot

Over-treatment can leave a pale dot, which recovers slowly.

Darker skin

Both of the above are more likely in darker skin. Conservative settings and sun protection reduce the risk but do not remove it.

Misdiagnosis

A changing mark could be lasered as a freckle. This is why diagnosis comes first.

Uncommon effects

Blistering, pinpoint bleeding and cold sore flares. Report these to the clinic the same day.

Sun care and fees

Daily broad-spectrum sunscreen, a hat and midday shade all matter. A topical retinoid or other prescription lightening agent may be discussed after assessment. Where freckles sit alongside sun spots or uneven tone, the T2 Frax Radiance protocol may be considered. Fees depend on the area, the number and density of spots, the wavelength and mode, any test area and how treatment is staged. No prices are listed, as Singapore's rules do not allow them to be advertised. A written, itemised quote follows consultation.

Frequently Asked Questions

No. It treats the pigment already there. Sun prompts new freckles, and the laser does not change that tendency.
No number is promised. Treatment may be staged, and each step depends on how the skin healed from the last.
Treated spots go grey or white for minutes, and then a fine crust forms. It flakes off by itself. Do not pick it.
It is better to wait until the tan fades. Tanned skin absorbs more energy and darkens more readily.
It may be, with adjusted settings and often a test area. Dark marks and pale dots are more likely in darker skin, so sun protection matters.
It is reviewed, not retreated. Please contact the clinic so the area can be checked.
