The short answer
HIFU heats small points at set depths to prompt collagen remodelling, targeting the SMAS and dermis. Most effects afterwards are short-lived. The more serious ones arise when heat lands where it should not.
Depth, energy, coupling and nerve courses depend on the operator, not only the device. Ultherapy Prime uses imaging to show the layers before each line. Ultraformer MPT is micro-pulsed without imaging, so it relies on palpation and a depth map built at assessment.
HIFU side effects you can expect
Discomfort over bone at the jawline, cheekbone and brow. Most people describe a deep ache or brief jolt.
Redness, warmth, tenderness along treated lines, and some swelling along the jaw.
A bruise, or a patch of altered sensation.
Risks when heat lands in the wrong place
Heat near a nerve branch can affect feeling or movement.
Heat in thin fat can cause hollowing. A lean face is more exposed.
These are linked to energy delivered too superficially.
No rate is given here. A 2025 systematic review of adverse events found the evidence limited.
Who it does and does not suit
| Picture | View |
|---|---|
| Early to moderate laxity of the jawline, under the chin and neck | May suit, with enough tissue depth above bone |
| Sag from volume loss, such as a hollow midface or temple | Not advised |
| A full fat pad under the chin | Needs a fat-directed approach |
| Established skin excess | May need a thread lift or surgical assessment |
| Crepey skin without descent | Needs skin-quality work |
| Lean or volume-depleted faces | Deeper cartridge avoided, or HIFU declined |
| Older, sun-damaged, thin or smoker's skin | Remodelling is smaller and slower |
What it cannot do
HIFU cannot replace lost volume, reduce a full fat pad, remove redundant skin, change skin quality or treat pigment. Nothing meaningful is visible on the day. The response is gradual, variable and not fixed in size. Any change also depends on age, skin quality, the degree of laxity at the start, sun exposure, weight change, smoking and ongoing ageing. Weight gain or loss can undo tightening, and ultraviolet exposure degrades collagen continuously. No duration is promised.
Is it worth it for you?
That depends on whether HIFU matches the cause. It may be worthwhile if you have early to moderate laxity, a face that is neither markedly heavy nor depleted, and acceptance that change develops over months. It is unlikely to be if your sag comes from lost volume or excess skin. A consultation may end with no treatment advised, and the VF Lift, which uses monopolar radiofrequency rather than focused ultrasound, may be discussed as an alternative or add-on.
What to report to the clinic
Numbness or tingling that persists, or new weakness when smiling, pursing the lips or raising the brow.
A raised welt, linear mark or blister along a treated line that does not flatten.
Marked or one-sided swelling, pain that increases, or a new hollow over the cheek or temple.
Sessions, recovery and fees
Tenderness and some swelling are expected in the first days, and plans are timed around tenderness and chewing discomfort. HIFU downtime covers this in detail. Review is timed to the tissue's response. No repeat is sold in advance, and there is no fixed repeat interval. Systematic reviews of microfocused ultrasound show that follow-up periods and protocols vary widely, so published results are hard to apply to one face. Fees depend on the area treated, cartridge depths, number of lines, and whether HIFU is combined with radiofrequency or laser. Singapore's rules prevent clinics from advertising prices, so a written, itemised quote follows consultation.
