Nasolabial Folds Treatment Singapore

In Brief

Nasolabial fold treatment targets what deepens the smile lines between the nose and the corners of the mouth. Dr Sin Yong looks at the cheek above the fold first. Laxity and sagged fat may suit Time Freeze Laser LCLR®, a lean face the VF Lift, and an etched crease a small amount of filler. Results vary.

What the Research Shows: Nasolabial Folds Treatment

  • A cadaver study of 30 hemifaces found facial fat divided into separate compartments, including a discrete nasolabial unit. [1]
  • A dissection study found that the fold varies in type, and described it as a conflict between the skin and fat envelope and the muscles beneath. [2]
  • A review describes ageing across bone, ligaments, muscles, fat and skin, with the rate varying by structure and by person. [3]
  • We found no published study of Time Freeze Laser LCLR® on the nasolabial fold. It is used here on clinical judgement, and response varies.

About This Concern

Why the fold deepens

The nasolabial fold is where the mobile cheek meets the fixed upper lip, and it creases every time you smile. Seeing it in expression is normal at any age. What changes with time is how it looks at rest.

The cheek holds fat in separate compartments, and these change with age. As the cheek deflates and descends, tissue piles against the fold from above. So the fold is usually the shadow of something happening higher up.

That is why Dr Sin Yong starts with the cheek.

Which type do you have?

Most faces have more than one of these. Marionette lines, which run down from the mouth corners, are assessed separately.

How Time Freeze is used on the fold

Dr Sin Yong uses Time Freeze Laser LCLR® on nasolabial folds in two ways. Heat at several depths of the dermis is directed at laxity across the cheek and around the fold. Where fat has sagged beside the fold, the Sculpting mode is directed at that fat in zones he selects.

Nothing is injected, so there is no product to add weight or to settle. Lean parts of the face are left out of the Sculpting mode, because heating fat in an area with none to spare can deepen hollows. Time Freeze does not replace lost volume, and if the cheek has deflated, that is planned separately.

Where filler fits

Filler has a place, but usually at the end. Placed in the crease alone, it can add bulk where tissue is already gathering. Dr Sin Yong restores support higher up first, and he uses filler sparingly where an etched crease remains.

He also looks at what is already there. Earlier filler can make the mid-face heavy, which makes the fold look deeper. In that case the first step is ultrasound mapping, and the excess is then targeted. See pillow face treatment.

Marionette lines

Marionette lines run down from the corners of the mouth, and they are a separate question from the fold. Lower-face descent, a jowl beside the line, and mouth corners pulled down by muscle can all play a part. Lifting comes first where descent drives it, and Time Freeze or the VF Lift may be considered. Botulinum toxin may be considered for muscle pull, and any filler is kept conservative. See jowls treatment.

Who should wait or be referred

Should wait

Pregnancy, freshly tanned or sunburnt skin before laser, infected or inflamed skin, including an active cold sore, and a recent course of oral acne medication.

Discuss first

Recent filler or threads in the area, keloid tendency, melasma and photosensitising medicines.

Referred on

Marked skin excess or heavy descent goes to a plastic surgery specialist.

See a doctor the same day, or go to A&E if none can see you

A fold that has become swollen, tender or uneven without a clear reason.

What treatment cannot do

Remove the fold

Every face has one, and it is part of how the face moves.

Fix a structural fold with cream

No topical reaches its depth.

Remove excess skin

Marked laxity needs a surgical opinion.

Replace lost volume with heat

Heat does not add volume. A deflated cheek needs support.

Risks and side effects

Laser

Warmth, tenderness where fat was targeted, and less often small crusts or temporary darkening. Rarely a burn, blister, lightening, scarring or more fat loss than intended. Eye injury if eyes are unprotected.

Filler

Swelling, bruising and lumps. Rarely a blocked blood vessel, which is an emergency. The area around the nose carries particular vascular risk. Skin turning white, grey or mottled, or worsening pain after filler: urgent assessment by the injector or A&E now. Change in vision or eye pain after filler: call 995. See the warning signs.

Radiofrequency

Warmth and redness that settle. Rarely a burn. A follow-up of 290 patients lists fat atrophy among less frequent reactions. [4] See the VF Lift page.

What affects the fee

Fees are not published here. They depend on the treatment chosen, the number of zones and how many steps the plan has. A written quote follows the assessment.

Frequently Asked Questions

The fold is a natural crease where the mobile cheek meets the fixed upper lip. It deepens with age as the cheek deflates or descends and tissue piles against it from above. Sagged fat beside the fold, sun damage and weight loss can all add to it.
They are the same thing in everyday speech. Smile lines or laugh lines usually mean the nasolabial folds, which run from beside the nose to the corners of the mouth. Marionette lines run down from the corners of the mouth, and they are assessed separately.
It can be used where the fold is deepened by laxity or by sagged fat beside it. Time Freeze Laser LCLR® heats the dermis and targets superficial fat in selected zones. It does not fill a crease, and it does not replace volume lost from the cheek.
Sometimes, as a finishing step. Filling only the line can add weight where tissue is already piling up, and it can flatten the transition from cheek to lip. Dr Sin Yong looks at the cheek above first, and any filler at the fold is kept conservative.
No. Every face has this fold, and it is part of how the face moves. Treatment aims to soften how deep it looks at rest. How much changes varies, and flattening a fold too far can look unnatural.
Neither reaches the cause. The fold is anatomy, not a weak muscle, and no cream reaches its depth. Sun protection and a stable weight are worthwhile, because they help to protect the skin and the cheek above the fold.
Filler placed in or above the fold can add weight, or it may have settled. A heavy, flat mid-face can make a fold look deeper. Ultrasound shows what is there. Hyaluronic acid excess is usually dissolved, and other settled product may suit the Sculpting mode after mapping. More filler is rarely the first step.
Yes. Where marked skin excess or heavy descent drives the fold, non-surgical treatment will have limited effect. Dr Sin Yong says so and refers you to a plastic surgery specialist for an opinion.
